Please complete this form as fully and as honestly as possible.  If you are a member of the volunteer’s family or on the same SU activity as them, please DO NOT complete this form.

Please bear in mind that we are looking for volunteers:

Who love Jesus and are committed to following him
Who care about helping children and young people know God more
Who are involved in a local church

Information you give regarding health, abilities or criminal convictions is considered highly important as it may affect the applicant’s suitability. We therefore ask that you give absolute priority to the safety and needs of the children and young people we are serving. We assure you that anything you write will be treated as confidential.
